• Designated Beneficiary - This document supersedes a Last Will & Testament. Make sure to update for major life changes.
• Nomination of Custodian For Minor (Custodian must be assigned if Beneficiary is a minor)

PERS Change of Address - Complete the highlighted personal information sections and send the form to HR. If you don't know your ID, leave it blank. Do not sign.
